Together we'll grow and sustain this rewarding, enriching environment for both our older adults and our employees.Living Independence For the Elderly (LIFE) is a managed care program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in the community through supportive services. The LIFE centers provide a single hub for primary care, pharmacy, nursing, restorative therapy, adult day services, care coordination and social work. The center staff coordinates additional supportive services such as home health care, transportation, home accessibility modification and home delivered meals. LIFE coordinates and provides all needed preventative, primary, acute and long-term care services. All of the team members work together to allow participants remain in their own home.The Activity Associate I works under supervision with general exercise of independent judgment in the planning, coordinating, and implementing of a comprehensive multi-level recreation program to meet the needs of participants.
