Job Description:

Encompass Health Corporation is looking for a dedicated and skilled Case Manager to join our team of healthcare professionals. The ideal candidate will manage a wide range of medical, social, and personal needs of our clients, coordinating between healthcare services to ensure high-quality care and efficient service delivery in our inpatient rehabilitation hospitals, home health, and hospice settings. This role is vital in advocating for patients, facilitating proper treatment plans, and ensuring each patient achieves the highest possible level of independence and quality of life. Assess client needs and develop individualized care plans in collaboration with healthcare professionals. Coordinate all phases of care from admission to discharge or transfer to other care facilities. Serve as the primary communication link between patients, family members, and multidisciplinary healthcare teams. Ensure compliance with healthcare regulations and internal policies. Monitor and report on the progress of clients and adapt care plans as necessary. Educate patients and families on healthcare rights, diseases, treatment plans, and healthcare resources. Strong interpersonal and communication skills In-depth knowledge of healthcare regulations and care coordination Ability to work independently and as a part of a team Effective problem-solving and critical thinking skills Experience with healthcare management systems Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) credential preferred.
