Current Physical Therapy license in Pennsylvania required to start the assignment. Current CPR certification required. Valid driver’s license, vehicle insurance, and access to a dependable vehicle for travel within the agency service area. Experience providing home health physical therapy or equivalent outpatient/home-based clinical experience preferred. Ability to complete patient evaluations, develop plans of care, and document per CMS and agency requirements. Schedule flexibility to participate in weekend rotations and potential availability on observed holidays as needed.
