Deliver skilled physical therapy and rehabilitation therapy in
patients’ homes - Develop and implement individualized treatment
plans - Collaborate with an interdisciplinary care team - Document
patient care using EMR system (HCHB) Qualifications - Licensed
Physical Therapist in Pennsylvania - Minimum 1 year of clinical
experience - Valid driver’s license and reliable transportation
