Job Description:

Encompass Health Corporation is seeking a dedicated Case Manager to join our diverse team, focused on providing exceptional facility-based and home-based patient care across our network. The Case Manager will be instrumental in planning, managing, and coordinating patient care to ensure the highest quality of healthcare services. This role involves working closely with patients, families, and multidisciplinary team members within our inpatient rehabilitation hospitals and home health as well as hospice agencies. Assess patient needs and develop individualized care plans Coordinate with healthcare providers to implement care plans Monitor patient progress and adjust care plans as needed Provide resources and support to patients and their families Ensure compliance with healthcare regulations and standards Maintain accurate patient records and report on case management activities Strong communication and interpersonal skills Excellent organizational and time management abilities Proficiency in healthcare software and system Ability to work collaboratively in a multidisciplinary team Knowledge of healthcare regulations and policies Medical Insurance Dental Insurance Vision Insurance 401k Licensed Registered Nurse (RN) or equivalent Certification in Case Management (CCM) is preferred
