Job Description:

Encompass Health Corporation, a leader in integrated healthcare services, is seeking a dedicated Case Manager to join our multidisciplinary team. This role focuses on providing personalized care coordination and advocacy for patients through our network of inpatient rehabilitation and home health services. The Case Manager will be instrumental in designing and implementing highly efficient treatment plans that enhance patient outcomes, ensuring a seamless transition between phases of care and optimizing the patient recovery journey. Assess new patients, identifying their needs and goals to develop tailored care plans. Coordinate with healthcare professionals to streamline the delivery of healthcare services. Monitor patient progress, making adjustments to care plans as needed. Maintain comprehensive documentation of patient care and communicate updates to all relevant parties. Educate patients and families on treatment processes, healthcare options, and preventative measures. Advocate for patient needs and preferences with healthcare teams. Strong organizational and communication skills Ability to work independently and as part of a team Knowledge of healthcare regulations and patient rights Experience in case management software and patient care coordination Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred Licensed Registered Nurse (RN) or Master’s in Social Work (MSW) based on the specific role and responsibilities
