Job Description:
The LPN/LVN administers skilled nursing care to terminally ill
patients according to the care plan and state Nurse Practice Act.
Provides direct bedside care, evaluates patient/family needs, and
assists in developing and implementing interdisciplinary care
plans. Performs treatments, medication administration, and monitors
patient reactions under RN supervision. Documents clinical notes,
reports significant changes, and maintains a safe environment.
Participates in patient/family education and supports their
independence. Assists in designing and delivering care, initiating
comfort protocols, and participating in professional growth
activities. Works flexible hours, including weekends and on-call
rotations. Requires current state nursing license, one year of
relevant experience, reliable transportation, and physical ability
