FULL AND PART TIME IN-HOME COMMUNITY BASED CASE MANAGER - CLEARFIELD COUNTY, PA

Family Resource Specialist



JusticeWorks is looking for a Full and Part Time In-Home Community Based Case Manager immediately in Clearfield County, PA. The qualified In-home Community Based Case Manager will be an independent, self-reliant person who appreciates being supported by a strong team.



To provide excellent service, our families are our priority so working with their schedules is key to creating a healthy rapport with them. Evenings and/or weekends may be required to fulfill a 40-hour work week.

The qualified In-home Community Based Case Maanger will have:


Bachelor's degree or high school diploma and experience working with children, youth, and/or families in a variety of settings or environments.
Valid driver's license, auto insurance and access to a vehicle.
Pass current, applicable clearances.
Pass pre-employment, post-offer drug screening.
Ability to work non-traditional hours including evenings and weekends.

The qualified In-home Community Based Case Manager will:


Assess and engage families in service planning based on demonstrated needs.
Plan and implement appropriate interventions using evidence based and best practice approaches.
Assist families in accessing and navigating community resources.
Attend court hearings and provide testimony as required.
Transport clients utilizing personal vehicle, as necessary.
